1. Type and weight of the respirator selected for employee
2. Duration and frequency of respirator use (including use for rescue and escape)
3. Expected physical work effort.
4. Additional protective clothing and equipment.
5. Temperature and humidity extremes that may be encountered.
6. Copy of the company's written respiratory program and a copy of CFR1910.134 Sec. (3)
